We want to make seeing your provider as easy and convenient as possible. Now you can schedule your own appointment online, check in and complete many of your forms several days before your appointment using your smartphone, tablet or computer.

That’s right, fewer paper forms to fill out!

• Book your appointment online through our website based on the reason for your visit.
• Starting 5 days before your appointment (or sooner for same-day or next-day appointments), you’ll receive a text or email with a secure link to check-in.
• Follow the instructions for providing the information we need for your upcoming appointment.
• You’ll also have the chance to update your medical information, pay your co-pay, and sign any necessary forms.
• When you arrive at our office for your appointment, scan our QR code in the waiting room to complete your check-in and notify us that you are here.

That’s it! Our new system is designed to empower you with greater self-scheduling functionality, reduce wait times, and maximize the quality time spent with your provider at our office. Please pay attention to your texts and emails to check in for future appointments and look for the orange check in signs when you enter our office.

Make sure we have your accurate cell phone number and email on file for our communications, and please bring your personal device to your appointment to complete check-in (and scan the QR code). Rest assured, our team continues to be here to assist you. You won’t be on your own. We will still have our front desk staff to welcome you, and we can provide you with a tablet if you don’t have a personal device or assist you through the whole process if you can’t check-in independently.

Be sure to follow the instructions in your appointment reminders, and continue to come to your appointment early if you are unable to check-in ahead of time so we can assist you with the check-in process without delaying your appointment.
