Effective March 13, 2024, Community Care Physicians will no longer participate in Humana. If you have this insurance, we are not discharging you from our practice and will continue to see you, but you should select another insurance to avoid paying out of pocket for your care. We accept many insurance plans from which you can choose that offer comparable coverage. View the letter sent to our Humana patients.
Community Care Physicians participates with many health insurance companies and managed care programs.
It’s important to know that your co-payments and coverage are determined by your insurance, not CCP. Before scheduling an appointment, it’s best to contact your insurance plan to confirm coverage for the services you seek and to understand your benefits and any out-of-pocket expenses required by your insurance plan (including deductibles, co-payments, and co-insurance).
Co-payments and Balances
Kindly note, co-pays are due at time of service.
If your insurance requires that you pay a co-payment, they require us to collect it. If we must bill you after your appointment, an additional processing fee may be charged. If you are unable to pay at time of service, we ask that you call our office ahead of time to make payment arrangements. We are happy to assist with a payment plan for balances due. If you do not have insurance, you can contact us to request an estimate of expected charges before receiving services and inquire about our self-pay discounts and payment plans.
